2017-2018 NOMINATION FORM
University Graduate Scholars Award
Departments and programs are invited to submit names and dossiers of nominees for the University Graduate Scholars Award to the Office of Graduate and Professional Education. Nominees for this award may be students newly admitted for the 2017-18 academic year or currently enrolled students. Nominees must be US citizens or permanent residents. Awards are competitive and are based on academic achievement as well as membership in historically underrepresented groups (African-American, Hispanic, Asian, Native American, two or more races, Native Hawaiian, or other Pacific Islander); physical disabilities; need as determined by federal income guidelines [FAFSA], or first-generation college student). University Graduate Scholar awards may be administered as graduate fellowships or graduate assistantships. Awards are for one academic year only, though students may be nominated for a second year.
Deadline December 15, 2016
Submit the following as a single pdf
- Completed nomination form for each nominee.
- A letter of nomination for each nominee from either the chairperson of the department or the director of the graduate program describing the accomplishments and suitability of the nominee for this award.
- New Nomination: a copy of the student’s personal statement. OR
- The c.v. of the nominee, including a listing of publications authored or coauthored, conference presentations and other professional accomplishments and activities.
Renewal Nomination: an evaluation letter from a faculty member describing the nominee’s academic performance and potential professional contribution to the field of study.
Submit all supporting documentation as a single PDF.
